National Program

Procedure for including eligible beneficiaries in the subprogram:

1. the requesting couple shall submit to the premises of one of the health units implementing the sub-program a file containing the following documents:

1.1. application for inclusion in the IVF / ET sub-program signed by the members of the couple, which must include the following data: identification data of the members of the couple: name and surname, personal numerical code, type of identity document, series and number of identity document, date of issue and issuing unit;

• Mobile phone number;
• Mailing address;

1.2. Copies of the identity documents of the members of the couple; 1.3. Certificates issued by the health insurance company showing the quality of insured in the social health insurance system of the couple's members, original documents, issued no later than 60 days before the submission of the file or any other documents according to law, proving the quality of secure; 1.4. Medical certificate issued by the family doctor showing the height, weight and body mass index of the woman - document issued no later than 60 days before the submission of the file; 1.5. Medical documents showing the minimum set of investigations performed to establish the diagnosis, therapeutic indication and medical risks of the requesting couple according to Annex no. IV.11 to this Annex to the Technical Regulations, which forms an integral part thereof; 1.6. A medical document from which to result the medical indication for performing the IVF / ET procedure, according to the provisions of annex no. IV.10 to this Annex, issued by a specialist in obstetrics-gynecology with competence in the treatment of couple infertility and medically assisted human reproduction, issued no later than 60 days before the submission of the file; 1.7. Declaration on their own responsibility stating that the applicants have submitted a single file at the headquarters of a single health unit running the IVF / ET subprogramme and that they have not benefited in the past from another procedure financed from the budget of the Ministry of Health; the statement must state the statement “Under the sanctions applied to the act of forgery and use of forgery in public documents, according to the provisions of the Criminal Code, I declare that the data in the statement are correct and complete. ”

1.8. Description of the file documents;

2. The medical registrar verifies the existence of all the documents in the file according to the description, compares the copies of the identity documents with the original documents and attests by signature the conformity;
3. The complete file containing all the documents referred to in point 1 shall be recorded in chronological order in a register of the documents of the IVF / ET procedures within the subprogramme, drawn up, completed, kept and archived by the health unit; the registration number assigned by the health unit for the submitted file shall be communicated in writing to the applicants;

4. The files submitted within one month are evaluated by a commission of the health unit that meets in the first 5 days of the current month for the previous month. The composition of the commission is established by the decision of the legal representative of the health unit and will have at least three members, including an obstetrician-gynecologist with competence or certificate of complementary studies in the field of couple infertility treatment and medically assisted human reproduction and an embryologist with European certification. in the field. The result of the evaluation of the files shall be recorded in a report which, after being signed by all members present of the commission, shall be recorded and kept at the health unit.
5. The Commission shall approve the files which meet all the legal conditions for inclusion in the sub-program, in the chronological order of the registration of the files and within the limits of the funds available for this purpose. If the applications for inclusion in the subprogramme
6. IVF / ET exceed the approved funds, the commission will draw up a waiting list that is registered and kept at the headquarters of the health unit. The list shall be regularly updated either by including new approved dossiers or by excluding dossiers in one of the following situations:

• The couple benefited from the IVF / ET procedure;
• The ovarian stimulation protocol was not initiated within 90 days of the registration of the commission's decision to include the couple in the sub-program at the health unit;
• The couple gave up the IVF / ET procedure with the written notification of the health unit;
• The result of the evaluation of the dossier shall be communicated in writing to the correspondence address recorded in the application referred to in point 1, as follows:

a) approved file with the date of appointment for the initiation of the IVF / ET procedure; b) file approved and registered in the waiting list specifying the serial number; in case the allocated funds allow the initiation of the IVF / ET procedure, the health unit notifies the requesting couple about the date of appointment; c) file not approved with the motivation of this decision. The communication of the result of the evaluation of the file is made in the form of a decision signed by the members of the commission and countersigned by the legal representative of the health unit or by his proxy. The decision is recorded in the register of documents of the IVF / ET procedures within the sub-program, the date of the health unit stamp is dated and applied. The copy of the decision and the confirmation of its transmission are kept and archived at the health unit.

7. If the ovarian stimulation protocol prior to the IVF / ET procedure is not initiated within 90 days of the registration of the decision at the health unit, the decision loses its validity.
